Telehealth Service in Newham
The Telehealth Service in Newham is provided by East London NHS Foundation Trust.
The service works closely with other Extended Primary Care Team services like rapid response, district nursing, physiotherapy, occupational therapy, care navigators and other specialist services to provide an integrated holistic approach to patient care.
You can get a referral through your GP or a healthcare professional. You can contact the team on 020 8536 2335 for more information.
There are three levels of monitoring:
1. Full vital sign monitoring called Docobo Telehealth
This is aimed to meet the needs of patients who:
- Have co-morbidities, such as a diagnosis of Heart Failure or COPD (a breathing condition)
- Need help with learning self-care skills and be closely monitored at home.
It is provided via personalised care plans using a remote monitoring service. Patients are loaned monitoring equipment, such as blood pressure machine, oximeter, weighing scale (for heart failure patients) and small touchscreen device to enter readings and symptoms.
2. Florence
Florence uses SMS text and mobile phones to monitor vital signs.
3. Guide
Patients are reviewed on a monthly basis over the phone.
The readings go to a centre where they are automatically logged and checked by a healthcare professional. If there is any cause for concern, you will be contacted and given advice or visited accordingly.
The service is available 7 days a week with a limited service over the weekend and bank holidays.
Evaluating the Telehealth Service
It is important that we closely monitor how effective Telehealth is and how people feel about it.
Everyone involved in the care of the person using the service should be kept informed and can contribute to the overall understanding and management of care. This includes:
- Families
- Carers
- GPs
- Other health professionals
- The individual themselves.
