Reablement case study
Resident CE was referred to the Newham reablement service by the hospital social work team.
The reablement service worked with CE to devise a plan that would meet their needs.
CE's goals were to be able to independently:
- Manage personal care
- Maintain a tidy home
- Manage medication appropriately.
Staff would:
- Supervise CE in the bathroom and make sure CE safely used the equipment provided
- Support CE to dress appropriately and change clothes on a daily basis
- Support and encourage CE to maintain a good standard of cleanliness in the home, including daily housework tasks, supervised by staff.
CE also had a self-medication assessment which identified that the use of blister packs would help CE manage their medication independently. At the end of the reablement intervention, CE had achieved their three goals.
CE explained: “I had just come out of hospital after being in hospital for approximately 8 weeks. I don’t think I would have managed without the reablement service. The staff encouraged me to carry out household chores and I can now do these for myself.”.
