Rapid Response Team

BSL Support

The Rapid Response Team provides triage, assessment and treatment in a patient’s home within two hours of the initial referral.

This enables patients to remain safe in their own homes under the care of their GP and avoid hospital admissions if they become acutely unwell.

The Rapid Response Team is made up of:

  • Advanced Nurse Practitioners - who have advanced assessment skills including the diagnosis of acute conditions and the prescribing of medications
  • Nurses
  • Physiotherapists
  • Occupational therapists (Assessments may include the provision of urgent equipment)

Referrals can be made by:

  • Yourself
  • Relative or carer
  • GP
  • London Ambulance Service
  • Any other health professional

Rapid Response Team

The service is open 7 days a week between 0800 – 2200 hours. Last referrals accepted at 2000 hours to allow for the 2 hours response.
